Child Strengths and Difficulty SDQ Assessment

If you are doing this questionnaire, it means you’re taking the time to reflect on your child’s emotional and behavioural wellbeing. Your observations play an essential role in understanding how your child is feeling and functioning in everyday life.

Instructions

This Parent’s version of Strengths and Difficulties Questionnaire (SDQ) is a structured behavioural screening tool designed to assess the following for children between 4 – 12 years:

1. Emotional symptoms (e.g., worries, sadness, fears)
2. Behavioural regulation (e.g., temper, rule-following)
3. Attention and activity levels
4. Peer relationships
5. Prosocial behaviours such as kindness and helpfulness

You will be presented with a series of statements about your child. For each statement, please select each of the following to the best of your knowledge: Not True, Somewhat True, Certainly True.

Please answer based on your child’s behaviour over the past 3 – 6 months.

This questionnaire is not a diagnostic tool. Instead, it provides structured screening results indicating whether your child’s scores fall within typical ranges or suggest areas that may benefit from further professional discussion.  The assessment typically takes 5–7 minutes to complete. Please answer as honestly and accurately as possible based on your observations.

Child Difficulties Score interpretation

The Child Difficulty Assessment utilizes a standardized scale to categorize results:

• 0 – 13 (Normal) 

• 14 – 16 (Borderline)

• 17 – 40 (High) 

Child Strengths (Prosocial) Score Interpretation:

The Prosocial subscale assesses the child’s positive social interactions. 

• Normal (6–10)

• Borderline (5)

• Low (0–4)

This data is used solely to assess your child’s mental well-being and provide tailored feedback.
